Usually one of three things: dead skin cells that shed too slowly and build up on the surface, pores clogged with oil and debris, or marks and shallow indents left by old breakouts. The AAD notes that clogged pores, oily skin, and acne can all make pores look larger, and that inflamed skin makes pores more noticeable. Your provider will work with you to figure out what’s behind your texture and build your formula around it.
It can make them look smaller. Pore size is mostly genetic, so you can’t permanently shrink pores, but keeping them clear of oil and debris helps them look smaller. Tretinoin clears that buildup, and niacinamide helps with oil: in a placebo-controlled study, 2% niacinamide significantly reduced oil production and pore size over four weeks. Results build over a few months of consistent use.
It might. Tretinoin speeds up cell turnover, which can bring clogged pores to the surface as small breakouts in the first few weeks. That’s normal and usually settles as your skin adjusts. If it lasts longer than about six weeks, or your skin feels very irritated, message your care team. Your provider can lower your strength or add a soothing ingredient.
Most people notice smoother, more even skin in two to three months, after an adjustment period of a few weeks. Retinoids speed up the turnover of surface skin cells and boost collagen, so results build over time rather than overnight. Keep using your cream consistently, wear sunscreen daily, and your provider will adjust your formula as your skin responds.
It depends on the scar. The AAD says that for mild acne scarring, a retinoid can make scarring less noticeable. A custom cream can also improve shallow texture, uneven tone, and the dark marks left after breakouts. Deeper or pitted scars usually need in-office treatment: dermatologists use lasers and microneedling to rebuild collagen in scarred skin. Your provider will be honest about what a topical can do for your skin.
Yes. Your provider can start you on a lower strength, use a more hydrating base, and add ingredients like niacinamide to support your skin barrier. Most people should pause scrubs and acid toners while starting tretinoin, since stacking them increases irritation. The AAD also notes that scrubbing can irritate your skin, which can make your pores look larger. Your provider will tell you which parts of your routine to keep.
