Isotretinoin is the most effective oral treatment for persistent acne. Prescribed online by Honeydew dermatologists with close monitoring, safety oversight, and medication delivered to your door. Available as Accutane, Microdose Accutane, and Absorica.
Eligible patients may be prescribed Accutane-branded isotretinoin or other equivalent generic versions depending on pharmacy selection, insurance coverage, and pharmacy stock.
IMPORTANT SAFETY INFORMATION FOR ACCUTANE® (ISOTRETINOIN): Accutane® must not be used by patients who are or may become pregnant. There is an extremely high risk that severe birth defects will result if pregnancy occurs while taking Accutane in any amount, even for short periods of time. Potentially any fetus exposed during pregnancy can be affected. There are no accurate means of determining whether an exposed fetus has been affected. Read more about serious warnings and safety info.
Isotretinoin treats mild to severe, cystic, nodular, hormonal, and treatment-resistant acne by targeting excess oil production, clogged pores, inflammation, and acne-causing bacteria.
Dry lips, skin, and eyes are very common side effects of Isotretinoin. Some people experience temporary acne flares (also known as the Isotretinoin purge), muscle aches, or fatigue. Your provider closely monitors side effects throughout treatment.
Isotretinoin is regulated under the iPLEDGE program. Patients who can become pregnant must follow strict pregnancy prevention requirements, including monthly pregnancy testing. All patients complete regular safety check-ins before refills.
Honeydew makes Isotretinoin treatment simple and fully online. Your dermatologist manages your Isotretinoin dose, labs, and safety monitoring, while we coordinate iPLEDGE requirements, prescriptions, and delivery so you can focus on your skin, not paperwork.
Isotretinoin is available under several brand names and in different dose formulations. Your Honeydew dermatologist will recommend the option that fits your acne severity, lifestyle, and treatment goals.

Yes. Isotretinoin is the generic name for the active ingredient, and Accutane was the original brand name. Accutane is no longer sold in the U.S., but the name has stuck. Most people still use "Accutane" and "isotretinoin" interchangeably. When your dermatologist prescribes isotretinoin, you may receive it under a brand name like Absorica or as a generic, depending on your pharmacy and insurance coverage.
Isotretinoin and tretinoin are both retinoids derived from vitamin A, but they work differently and treat different levels of acne. Isotretinoin is an oral medication used for moderate to severe, cystic, or treatment-resistant acne. It works systemically, shrinking oil glands throughout the body. Tretinoin is a topical cream or gel applied directly to the skin, used for mild to moderate acne and skin texture concerns. If other treatments including topical tretinoin haven't cleared your acne, isotretinoin is typically the next step.
Mood changes are listed as a possible side effect of isotretinoin, and it's one of the most common questions patients have before starting treatment. The current medical evidence is mixed — large studies have not confirmed a direct causal link between isotretinoin and depression, and some research suggests acne itself (particularly severe acne) is associated with anxiety and low mood, which often improves as skin clears. That said, Honeydew takes this seriously: your provider will check in on your mood throughout treatment and will adjust your care if needed. If you have a history of depression or anxiety, make sure to discuss it with your provider before starting.
Some patients report temporary hair thinning or increased shedding while on isotretinoin, typically related to the medication's effect on cell turnover. In most cases this is temporary and resolves after treatment ends. It is not a common side effect for everyone. If you notice significant hair loss during your course, flag it to your Honeydew provider as iit may be worth adjusting your dose or timeline.
Yes, isotretinoin can be effective for hormonal acne, particularly when it is moderate to severe, cystic, or hasn't responded to hormonal treatments like spironolactone or oral contraceptives. While isotretinoin does not directly change hormone levels, it dramatically reduces oil production which is the mechanism through which hormonal fluctuations trigger breakouts. Your Honeydew provider will assess whether isotretinoin or a hormonal treatment is the better fit for your specific pattern of acne.
Isotretinoin is appropriate for adults of any age. While it's often associated with teenage acne, adult acne — particularly in people in their 20s, 30s, and beyond — is extremely common, and isotretinoin is one of the most effective options available when other treatments haven't worked. Adult patients are evaluated and monitored in exactly the same way as younger patients. Many Honeydew patients starting isotretinoin are adults.
Yes. Isotretinoin works systemically, meaning it reduces oil production throughout the body, not just on the face. It is one of the most effective treatments available for chest acne, back acne (sometimes called bacne), and shoulder acne, especially when breakouts are deep, cystic, or leaving scars. If body acne is your primary concern, mention it during your intake so your provider can factor it into your dosing plan.
Isotretinoin dosing is calculated based on your body weight, typically in the range of 0.5 to 1 mg per kilogram per day, split across two daily doses. Low-dose protocols use a lower daily amount over a longer period, while standard-dose courses run 6–12 months. Your provider will set your starting dose and adjust it based on your skin's response and any side effects. Total cumulative dose — the overall amount taken across your full course — is an important factor in long-term results. You can use our Cumulative Dose Calculator and Dosing Calculator to get a sense of your treatment trajectory.
The cost of isotretinoin depends on whether you have insurance, which pharmacy you use, and whether you receive a brand-name or generic version. Generic isotretinoin can be as low as $30–$50 per box with a GoodRx coupon, and as low as $0 with insurance coverage.
Honeydew's Accutane care membership starts at $50/month on an annual plan and covers your dermatology care, monitoring, and iPLEDGE coordination. Medication costs are separate and billed through your pharmacy. For a detailed breakdown, see our cost guide.
iPLEDGE is an FDA-mandated safety program that all patients, prescribers, and pharmacies must participate in before isotretinoin can be dispensed. It exists because isotretinoin can cause severe birth defects if taken during pregnancy. Patients who can become pregnant must complete monthly pregnancy tests and confirm two forms of contraception before each refill. All patients, regardless of pregnancy risk, complete monthly check-ins through the iPLEDGE system before their prescription is released. Honeydew guides you through every iPLEDGE step throughout your treatment.
